Indians are increasingly vulnerable to heart disease at a significantly younger age, often manifesting a decade earlier than in many Western populations. Prof. Rishi Sethi, head of cardiology at King George’s Medical University (KGMU) in Lucknow, attributes this trend to a complex interplay of genetic predisposition, environmental factors, and lifestyle choices. As the medical community prepares for a townhall on the issue, experts are emphasizing the urgent need for earlier screening and lifestyle interventions to combat what is being described as a growing cardiovascular crisis. This trend is driven by a combination of factors, including a genetic tendency toward visceral fat accumulation, which is often concentrated around internal organs rather than under the skin. This type of fat is strongly linked to hypertension, diabetes, and unhealthy cholesterol levels, all of which contribute to atherosclerosis—the narrowing of arteries that restricts blood flow. While these changes typically begin around age 55 to 60 in Western populations, clinicians are observing similar arterial damage in Indians as early as age 40. Prof. Sethi describes the phenomenon as a three-part mix of "bad genes, bad environment and bad lifestyle and diet." The genetic component is particularly concerning, as it predisposes many individuals to metabolic issues even in the absence of traditional risk factors. Visceral fat, often visible as abdominal or belly fat, serves as a primary marker for these underlying metabolic disturbances that damage blood vessels over time. However, experts stress that genetics are not the sole determinant. Poor diet, physical inactivity, tobacco use, obesity, and high blood pressure act as accelerators for arterial damage. Because the progression of heart disease is often silent, many individuals remain unaware of their condition until a major cardiac event occurs. Clinicians are now advocating for more targeted testing, such as measuring Lipoprotein(a) or Lp(a) levels, which are largely inherited and can identify individuals at higher cardiovascular risk even when standard cholesterol panels appear normal. Beyond adult cardiovascular health, pediatric experts are raising alarms regarding the nutritional status of children in Uttar Pradesh. Data from the National Family Health Survey (NFHS)-6 (2023-24) indicates that only 17.1% of children aged six to 23 months in the state receive an adequate diet. While this represents an improvement from the 5.9% recorded in NFHS-5, the vast majority of children—82.9%—still fail to meet essential nutritional standards. Prof. Shalini Tripathi of KGMU’s paediatrics department emphasizes that an adequate diet requires more than just sufficient calories; it necessitates a diverse range of food groups and consistent feeding schedules. The period between six and 23 months is critical for cognitive and physical development, and reliance on breast milk alone becomes insufficient after six months. Failure to provide a balanced, nutrient-dense diet during this window increases the long-term risk of stunted growth, infections, and delayed brain development. Addressing malnutrition remains a significant challenge, with stunting, wasting, and underweight prevalence rates fluctuating across the state. Prof. Shally Awasthi, former head of paediatrics at KGMU, identifies three primary barriers to child nutrition: money, knowledge, and time. Even when families possess the financial means to provide healthy food, a lack of awareness regarding proper dietary diversity or a lack of time for meal preparation can lead to suboptimal outcomes. Experts are urging parents to move beyond the convenience of readily available food and instead focus on integrating cereals, pulses, vegetables, fruits, and protein-rich sources like eggs into a child’s daily intake. Regular monitoring of growth metrics, such as height and weight, is recommended to ensure early detection of developmental issues. When a child’s growth deviates significantly from expected levels, consultation with a paediatrician or nutrition expert is advised to implement corrective dietary strategies. As the medical community in Lucknow intensifies its focus on these issues, the emphasis is shifting toward proactive management. Dr. Akshyaya Pradhan, who is moderating upcoming discussions at KGMU, has urged individuals with a family history of heart disease or other risk factors to prioritize preventive screenings. By engaging in early risk management, clinicians hope to mitigate the impact of premature coronary artery disease and improve long-term health outcomes for the population. Future efforts will likely focus on bridging the gap between clinical knowledge and public practice. Whether through public health campaigns or individual consultations, the goal remains to address the silent progression of cardiovascular and nutritional issues before they manifest as chronic disease. The upcoming KGMU townhall and film screenings, including "The Brown Heart Syndrome," serve as part of a broader initiative to educate the public on these critical health challenges.
