ICMR-NIN Study Identifies Diet-Exercise Combo as Most Effective for Sustained Weight Loss

Combining healthy dietary practices with consistent physical activity remains the most effective non-pharmacological strategy for achieving long-term weight loss and improving metabolic health, according to a comprehensive umbrella review conducted by the ICMR-National Institute of Nutrition (NIN). The findings, published in the international journal Obesity Pillars, offer a definitive look at how different lifestyle interventions impact body weight and cardiometabolic markers in adults struggling with overweight or obesity.
The research, led by scientists Mahesh Kumar Mummadi and Samarasimha Reddy, synthesized data from 112 systematic reviews and meta-analyses. This massive evidence base encompassed 2,332 randomised controlled trials and more than 5.07 lakh participant observations across over 50 countries. By comparing various approaches, the study provides a clear hierarchy of effectiveness for individuals seeking to manage their weight and improve their overall health profile.

Comparative Effectiveness of Weight Loss Strategies

The review established that while various non-pharmacological interventions lead to statistically significant reductions in body weight and Body Mass Index (BMI), the magnitude and sustainability of these results vary considerably. Low-calorie diets emerged as the most potent tool for rapid, short-term weight loss, yielding an average reduction of 2.64 kg within the first three months of implementation.
However, the study found that physical activity, while producing smaller initial weight reductions compared to dietary changes alone, offers superior benefits over extended periods. When diet and physical activity were combined, participants achieved the most sustained results, losing an average of 3.68 kg beyond the 12-month mark compared to control groups. This suggests that while diet drives early progress, the integration of exercise is critical for maintaining weight loss over the long term.

Metabolic and Cardiovascular Health Benefits

Beyond simple weight reduction, the ICMR-NIN review highlighted the profound impact of lifestyle interventions on metabolic health. The combination of diet and physical activity was identified as the most effective approach for improving blood sugar control, specifically by reducing fasting blood glucose levels and insulin resistance after one year of consistent practice.
Physical activity also demonstrated unique cardiovascular advantages. The data indicated that exercise is the most effective strategy for lowering blood pressure, with participants seeing a reduction in systolic blood pressure of approximately 6.1 mmHg between six and 12 months. While intermittent fasting showed promise for short-term diastolic blood pressure reduction and weight loss, these specific benefits were not sustained beyond the one-year threshold, highlighting the limitations of such focused dietary interventions in isolation.

The Role of Behavioral Support

Lifestyle interventions that incorporate behavioral components, such as goal-setting, self-monitoring, health education, and professional counseling, were found to be increasingly effective over time. These behavioral strategies act as a support system that reinforces the adoption of healthy habits, making them more likely to stick. The study suggests that these psychological and educational tools are essential for transforming short-term weight loss efforts into permanent lifestyle changes.
ICMR-NIN director Bharati Kulkarni emphasized that the findings underscore the necessity of an integrated approach to the obesity epidemic. "Healthy dietary practices, regular physical activity and behavioural support each have a role, but their combination can provide broader and more sustained benefits," Dr. Kulkarni stated. She added that this integrated model is particularly vital for India, where the rising burden of obesity and related non-communicable diseases requires scalable and sustainable public health solutions.

Limitations and Future Research Needs

Despite the scale of the review, the researchers noted a significant geographical bias in the existing body of evidence. While the study analyzed data from more than 50 countries, the vast majority of the underlying research originated from high-income and upper-middle-income nations. This leaves a notable gap in understanding how these interventions perform in lower-middle-income contexts, including India.
Mr. Mummadi noted that the current evidence base does not fully capture the nuances of diverse socioeconomic and cultural environments. Consequently, there is an urgent need for more locally relevant research to tailor these global findings to the specific needs of the Indian population. Future studies will need to focus on how to adapt these proven strategies to ensure they are accessible and effective for individuals across different economic backgrounds and regional settings.

Next Steps for Implementation

The ICMR-NIN has identified the need for further research to bridge the gap between global evidence and local application. While the current findings provide a clear roadmap for individuals and public health practitioners, the institute is expected to continue its focus on developing context-specific guidelines. There is no immediate change to clinical protocols, but the findings are intended to inform future public health strategies aimed at curbing the prevalence of non-communicable diseases in India. Researchers are now looking toward designing trials that specifically address the unique dietary and lifestyle patterns of the Indian population to refine these recommendations further.
